Enhanced Care Management

Starting Spring of 2024 Alameda Family Services began providing Enhanced Care Management (ECM) for community members living in or near the city of Alameda. ECM is a free benefit for Alameda County Medi-Cal recipients. Enhanced Care managers can support youth and their families up to 21 y.o and adults 21+ by providing an assessment of needs and care coordination to identify, manage, and connect clients to helpful services and resources. ECM supports people with life and health challenges such as mental, physical, and behavioral health issues, food insecurity, unstable housing, transportation challenges, employment and education assistance, interactions with law enforcement or crisis intervention due to mental health and/or substance abuse, new mom, and transitioning home from hospitalization. AFS' Lead Care Managers collaborate with healthcare professionals, coordinate and assist with appointments, provide accompaniment and transportation assistance, and offer tailored support to meet the unique needs of each individual receiving the service.

If you believe you qualify and would like to self-refer for services, please complete the AFS ECM Referral Form. Please note that authorization for services can take longer than expected, up to several weeks, and in some cases may be denied by the county.
