Alameda Family Services (AFS) is a progressive, non-profit agency that provides clinically sophisticated, comprehensive mental health and early childhood educational and family support services to individuals and families in the City and County of Alameda. Programs include: Early Head Start, Head Start, Family Support Services, School Based Services, and Behavioral Health Care Services (Counseling and an APA- accredited Narrative Therapy Training Program).  One of Alameda Family Services’ core values is fostering wellbeing. OVERALL RESPONSIBILITIES:

The CLinical Case Manager (CCM) will provide follow-up case management services to individuals of all ages experiencing a mental health crisis receiving emergency services through the City of Alameda’s new alternative to police response model. Please note that the Alameda Fire Department is the first responder in all cases with AFS playing a supporting role. The CCM will carry a caseload of clients referred by the Alameda Fire Department (AFD) and Alameda Police Department in the city of Alameda. Services will be intensive and short-term and delivered in the home, community, or office. Our ideal candidate would have previous experience working in a community-based program with severely mentally ill clients of all ages and know how to work as part of a team.

JOB DUTIES:

Responsibilities include, but are not limited to:

  • Visit clients at their home, in the community, or at our office.
  • Conduct social determinants of health assessment.
  • Provide linkage services to clients for 14-30 days with an emphasis on cultural responsiveness and compassionate care.
  • Collaborate with a circle of support/collateral contacts at least once a week.
  • Daily submission of case notes.
  • Complete all clinical documentation in a thorough and timely way.
  • Facilitate Team Meetings with client (as appropriate) and client’s team including professional and natural supports.
  • Attend client meetings and partnership meetings as directed, as well as all workshops, trainings and in-services.
  •  Attend and participate in clinical meetings with community resources (such as Psychiatrists, Social Workers, Physicians, Therapists, etc.).

QUALIFICATIONS, SKILLS AND EXPERIENCE

  • Masters degree in Social Work, Counseling Psychology required, AMFT or ASW with active BBS registration preferred.
  • 1 year relevant experience serving clients with mental health needs preferred.
  • Must have your own reliable means of transportation and be willing to commute to various settings to meet with circle of support and the identified client (i.e., family homes, Adult Residential Facilities, day programs, schools, hospitals, etc.).
  • Proof of current and valid car insurance with minimum $300,000 bodily injury and property damage combined coverage, or $300,000 Bodily Injury and $100,000 property damage coverage.
  • Ability to work full-time.

PHYSICAL JOB DESCRIPTION 

  • Ability to sit for at least 50% of the time and stand for 50%.
  • Lifting of books, manuals, and other documents weighing up to 30 pounds may be encountered in performing essential functions.
  • Position requires occasional evenings hours.

CONDITION OF EMPLOYMENT

  • A background check and clearance from the Federal Bureau of Investigation (FBI) and Department of Justice (DOJ) background prior to employment is required. 
  • Proof of Covid Vaccination and boosters required prior to employment.

 Alameda Family Services offers a very generous benefits program.  Based upon a 40 hour work week you will receive:

  • $28.85 - 29.81 per hour.
  • This position is non-exempt.
